Liability Waiver and Release of Claims - Dance Your Life Studio LLC
I, the undersigned participant, acknowledge and agree to the following terms:

Assumption of Risk
I understand that participation in dance, fitness, stretching classes, workshops, private lessons, performances, or any other related events (“Activities”) involves inherent risks, including but not limited to physical injury, muscle strains, sprains, falls, or other accidents. I voluntarily choose to participate in these Activities with full knowledge of the risks involved.

Release of Liability
I hereby release, waive, discharge, and hold harmless Dance Your Life Studio LLC, its owner Viktoriya Statska, instructors, assistants, employees, agents, and representatives from any and all claims, liabilities, demands, or damages arising from or related to my participation in the Activities, including but not limited to personal injury, illness, or property damage.

Medical Treatment and Responsibility
I certify that I am physically fit and have no medical condition that would prevent my participation in the Activities. I understand that it is my responsibility to consult with a physician before engaging in any physical activity. In case of an emergency, I authorize Dance Your Life Studio LLC to seek medical treatment on my behalf if necessary. I understand that I am solely responsible for any medical expenses incurred as a result of participation in the Activities.

Indemnification
I agree to indemnify and hold harmless Dance Your Life Studio LLC, Viktoriya Statska, and all affiliated parties from any claims, legal actions, costs, or expenses (including attorney fees) arising from my participation in the Activities.

Photography and Media Release
I grant permission for Dance Your Life Studio LLC to take and use photographs or video recordings of me during the Activities for promotional, advertising, or marketing purposes, including but not limited to social media, websites, and printed materials. I understand that no compensation will be provided and that I waive any right to inspect or approve the final media.

Acknowledgment of Understanding
By submitting this form I confirm that I have read the Liability Waiver and Release of Claims in full and agree to its terms voluntarily. I understand that I am waiving certain legal rights, including the right to sue. This agreement remains in effect from the date of acceptance until revoked in writing.
